Referral Process

We have a one step referral process. Just phone or fax the following information:

Patient's first and last name
Patient's demographics and insurance information including phone number
Brief description or reason for the infectious disease consult
All relevant medical records, labs, scans, etc..

We will verify insurance coverage, obtain all necessary precerts and/or referrals needed.

We will call the patient directly and make an appointment.

We will fax you, the referring physician, a copy of the consultation note after we see the patient.
